چکیده

This paper presents a new non parametric atlas registration framework, derived from the optical flow model and the active contour theory, applied to automatic subthalamic nucleus (STN) targeting in deep brain stimulation (DBS) surgery. In a previous work, we demonstrated that the STN position can be predicted based on the position of surrounding visible structures, namely the lateral and third ventricles. A STN targeting process can thus be obtained by registering these structures of interest between a brain atlas and the patient image. Here we aim to improve the results of the state of the art targeting methods and at the same time to reduce the computational time. Our simultaneous segmentation and registration model shows mean STN localization errors statistically similar to the most performing registration algorithms tested so far and to the targeting expert's variability. Moreover, the computational time of our registration method is much lower, which is a worthwhile improvement from a clinical point of view.

3D Reconstruction of Subthalamic Nuclei from MRI

In this paper, a novel 3D reconstruction of subthalamic nuclei is proposed for recognition and location analysis of subthalamic nuclei in the brain. The method utilizes a region based level set method to segment 2D subthalamic nuclei from two different MRI images. Via the location and registration of two subthalamic nucleus contours, each set of subthalamic nucleus slices from one T2-weighted M...

Automatic Subthalamic Nucleus Targeting for Deep Brain Stimulation. A Validation Study

In functional stereotactic neurosurgery, one of the cornerstones upon which the success and the operating time depends is an accurate targeting. The subthalamic nucleus (STN) is the usual target involved when applying Deep Brain Stimulation (DBS) for the Parkinson’s disease (PD). Unfortunately, STN is usually not clearly visible in common medical imaging modalities, which justifies the use of a...

Anatomical situation of the subthalamic nucleus (STN) from midcommissural point (MCP) in Parkinson\'s disease patients underwent deep brain stimulation (DBS): an MRI targeting study

Abstract   Introduction: It is demonstrated that the degree of clinical improvement in   Parkinson's disease (PD) achieved by deep brain stimulation (DBS) is largely dependent   on the accuracy of lead placement. In addition, individual variability in the   situation of subthalamic nucleus (STN) is responsible for spatial inter-individual   fluctuations of the real patient's target.   Objecti...

Feature-segmentation-based registration for fast and accurate deep brain stimulation targeting

Objects Deep brain stimulation (DBS) has turned out to be the surgical technique of choice for the treatment of movement disorders, e.g. Parkinson’s disease (PD), the usual target being the subthalamic nucleus (STN). The targeting of such a small structure is crucial for the outcome of the surgery. Unfortunately the STN is in general not easily distinguishable in common medical images. Material...

Validation of a fiducial-based atlas localization method for deep brain stimulation contacts in the area of the subthalamic nucleus.

Differences in the location of active contacts with respect to the subthalamic nucleus (STN) may account for much variability in motor, psychiatric and cognitive responses to deep brain stimulation (DBS) in Parkinson disease (PD) patients. Because localization of STN based on hypointensity in T2-weighted MR images is unreliable and further limited by artifacts from the metal electrodes, we deve...
